If you have a compliment, complaint, or other issue which you
believe requires resolution, please contact the Patient Advocate at (847) 688-1900 extension 83829. If you have any questions about your care, or if you are not
receiving the services you need, we encourage you to speak to
the members of your treatment team first. However, if
you prefer to discuss your problem with someone outside
your treatment team, you can call a customer service
representative. The appropriate customer service
representative will address your concerns. The Coordinator
of the Customer Service Representative Program is the Patient
Advocate and may be reached at (847) 688-1900 extension 83829.
