Chaplain / Religious Services
Our Chaplain Service will work with your Primary Care Team and/or Mental
Health Team to make sure you are visited and to support you in meeting your
spiritual needs while you are an inpatient. As an outpatient, you are also
allowed to visit with our Chaplains. To make an appointment with the Chaplain of
your choice, simply call the Chaplain Service directly at (847) 578-3715.

Hoptel
The medical center has lodging accommodations to allow you, your family, or
your caretaker a comfortable and safe environment in which to stay on a limited
basis while you undergo VA medical treatment.

Education Center
The Education Center is open Monday through Friday from 9:30 A.M. to 3:30
P.M. Books, magazines, large print books and videocassettes are provided to
support the recreational and educational needs of patients. A personal computer
is also available for our patients. The large collection of donated paperback
books do not have to be returned. Movies are shown daily.

Laboratory
The blood drawing area is open Monday through Friday from 7:15am to 4:30pm.
No appointment is necessary.
If you have any questions about your laboratory tests, please call Laboratory

Patient Education Resource Center
The Patient Education Resource Center (PERC) hours are 8:30 AM to 4:00 PM,
Monday through Friday. Health and wellness information is provided for patients.
This information consists of books, videocassettes and magazines, which promote
healthy behaviors, recovery and individual responsibility for health and helps
patients become more involved in their health care. Patient Education programs
are also broadcast on the medical center’s television, Channel 4
Pharmacy
Your prescriptions, written by VA providers, can be filled through the VA
pharmacy. The Outpatient Pharmacy is open Monday through Friday, 8:00 A.M. to
4:30 P.M. The telephone extension is 83752. There is a small co-payment for your
prescriptions, unless you are 50% or more service-connected. Refills and
renewals of your prescriptions will be mailed to your home. Please request the
refills as soon as you get the papers in the mail. You can use our telephone
refill system by dialing 1-800-393-0865 or (847) 578-3897 or by mailing in your
refill request. Be sure to keep us informed of any changes in your address or
telephone number. Often, we will substitute a generic drug, if it is comparable
to the brand your doctor prescribed. We can only fill prescriptions written on
official VA prescription forms. If you also visit a private physician, we regret
that we cannot fill prescriptions written by him or her. If your Primary Care
Provider agrees that you need that medication, he or she will rewrite the
prescription for that medication. Your Primary Care Provider is not obligated to
rewrite any prescription he or she feels is not in your best interest. If your
doctor does not agree to prescribe a certain medication, it is because he or she
cares about you!
Police and Security
Police and Security offers a number of services for you through the Community
Service program. The Community Service Officer can provide transportation from
the parking lots to the buildings as well as from building to building when
requested. He/She can also provide jump-starts for your vehicle’s battery as
well as a Lock-Out Service if you lock your keys in the car. Police & Security
also has a number of crime prevention programs such as self defense lessons.
VA Police Officers are authorized to enforce all Federal and State laws
including parking and traffic regulations of both VA and the State of Illinois.
Please obey all posted signs and Illinois vehicle laws. If you observe a
criminal or suspicious act, please notify the VA Police on our emergency
telephone number extension 911.
If you should need a service provided by the VA Police, please contact them
via the non-emergency extensions 83703, 83890 or 81486.
Coordination of Care
If further treatment services are not available within the VA Great Lakes
Health Care System, your Primary Care Team will discuss with you when and how to
obtain the treatment you need. Your team social worker or case manager can
assist you with your questions about available community resources and their
cost.
Protective Services
The medical center is concerned for the safety and well being of our patients
and their family members. If you have any concerns or questions about a
situation in which you feel you are being taken advantage of or in which you
feel unsafe, please share this with a member of your treatment team who will
refer you to your team social worker or case manager. Elder abuse, domestic
violence and child abuse are issues that impact the health and welfare of our
community members. Your social worker or case manager will provide you with
information and assist you in contacting the appropriate agency.
Veterans Benefits Counselor
The basic mission of the Veterans Benefits Counselor at the medical center is
to assist inpatients with filing claims and act as a link between the veteran
and the Regional Office. The Veterans Benefits Counselor's primary
responsibility is to inpatients but he/she is available to assist outpatients as
well.
Outpatient assistance is best accomplished directly through Veterans Benefits
Administration's toll free number, 1-800-827-1000. The Veteran Benefits
Counselor may be reached at extension 83822 or direct dial (847) 578-3822.
Veterans Service Organizations
Veterans Service Organizations (VSOs) are an adjunct to the Department of
Veterans Affairs. Representatives from several VSOs have offices in the

Women Veterans Committee
If you are a woman veteran, there is a Women Veterans Coordinator available
to assist you with any questions or concerns regarding your care at the medical
center. Pap smears, mammography, and gynecology referrals are available. Ask
your Primary Care Team for a referral. The Women Veterans Coordinator can be
